How we help
Expert Guidance for Rural Health Transformation Awards.
The Rural Health Transformation Program is a $50 billion federal initiative administered by the Centers for Medicare & Medicaid Services (CMS) that supports rural health care systems through fiscal years 2026–2030. The program is designed to strengthen rural health infrastructure, expand access to care, build a sustainable workforce, advance telehealth, and accelerate the transition to value-based care.
Successfully implementing a Rural Health Transformation award requires strategic planning, strong project management, regulatory compliance, financial oversight, and effective coordination among multiple stakeholders. We partner with health systems, hospitals, clinics, nonprofit organizations, and community partners to help them successfully manage and implement these complex awards.
Whether you are preparing to launch a new Rural Health Transformation initiative or need experienced support managing an existing award, we provide the strategic guidance, administrative expertise, and operational leadership needed to keep your project on track, compliant, and focused on achieving measurable outcomes for rural communities.
About the program
A $50 billion federal initiative, distributed to all 50 states.
The Rural Health Transformation Program is a $50 billion landmark federal initiative administered by the Centers for Medicare & Medicaid Services (CMS). It distributes funding to all 50 U.S. states to strengthen rural healthcare infrastructure, expand access to care, and combat chronic disease.
The federal program is designed around five core strategic goals:
01
Make rural America healthy again
Expanding preventive care, chronic disease management, and maternal health services.
02
Sustainable access
Helping rural providers coordinate operations to remain long-term community assets.
03
Workforce development
Recruiting and retaining clinical talent, including community health workers and doulas, in rural regions.
04
Innovative care
Sparking new, value-based care delivery models and regional collaboration.
05
Tech innovation
Investing in digital health tools, telemedicine, and enhanced cybersecurity.
Since states are empowered to direct funds based on local need, the program’s rollout varies by region. For example:
- New York: Allocated millions toward the Rural Community Health Integration Initiative to establish coordinated rural health partnerships.
- North Carolina: Directed funds toward “NC ROOTS” Hubs to seamlessly integrate medical, behavioral, and social services across counties.
- Missouri: Expanded Mobile Integrated Healthcare and Community Paramedicine, bringing clinical services directly to rural patients’ homes.
